What is Community Day Services?
Community Day services programs are offered to adults who have a developmental disability who live in residential settings, as well as by themselves or with a family. These programs include purposeful and meaningful activities and are designed to help individuals gain skills, teach appropriate behavior and coping strategies, build independence, and promote personal choice. Programs may include daily living skills instruction, community outings, vocational training, employment opportunities and recreation.

We believe clients play an important role within their local community, as well as the worldwide community, and deserve to take part and contribute through volunteer and potential employment opportunities.
CDS Goals

How to Apply?
An important first step is to register with Prioritization of Urgency of Need for Services (PUNS) database.
To register, you must meet with an Independent Service Coordinator Agency (ISC) for pre-admission screening for eligibility.
If you do not have an ISC, you can search the DHS Office Locator at: www.dhs.state.il.us for the ISC agency closest to you. Next to “county”, select your county. If you are in Cook County, you must select “Cook” and enter your zip code. The agency that shows can help you learn more about the DD service system.
